Jason W. Hendrix, MD
Dr. Hendrix is board-certified by the American Board of Ophthalmology. He was born and raised in Salt Lake City, Utah. He attended Brigham Young University where he met his wife, Kristyn, and graduated Cum Laude with a Bachelor of Art in History. He had the special opportunity of attending medical school at The George Washington University in Washington, D.C. After which, he completed his residency training in ophthalmology at the University of Missouri.
He has four wonderful children whom he loves spending time with. Soccer, piano, mountain biking and reading are a few of his favorite hobbies. Dr. Hendrix is a member of the Utah Ophthalmology Society, Utah Medical Association and a fellow of the American Academy of Ophthalmology. He enjoys humanitarian missions and recently returned from a trip to Ethiopia, where he performed eye exams and provided glasses to some of the world’s neediest.

D. Snow Slade, MD
Dr. Slade is board-certified by the American Board of Ophthalmology. He was born and raised in rural Arizona. He attended Brigham Young University where he graduated Summa Cum Laude (the top 1% of his class). After his undergraduate studies he went to medical school at the University of Arizona where he was selected for the Alpha Omega Alpha Medical Honor Society because of graduating at the top of his class.
He completed ophthalmology residency at the world-renowned Moran Eye Center at the University of Utah. He stayed an additional year at the Moran to complete a glaucoma and advanced cataract and intraocular lens surgery fellowship. He is currently an Adjunct Professor of the Moran Eye Center.
Dr. Slade is a member of the American Academy of Ophthalmology, the American Glaucoma Society, and the American Society of Cataract and Refractive Surgery. He has published in many prestigious ophthalmic journals and has enjoyed his humanitarian medical missions to El Salvador and Ghana.
He and his wife, Summer, are the parents of three children. He speaks Portuguese and Spanish. Dr. Slade enjoys restoring his Jeep, watching BYU sports and doing anything in the warm sun of Southern Utah.

Nicholas Behunin, MD
Dr. Behunin is board-certified by the American Board of Ophthalmology.
As a cornea specialist, Dr. Behunin is trained in the latest corneal transplantation techniques, Descemets Membrane Endothelial Keratoplasty (DMEK). He is also fellowship-trained in refractive surgery (LASIK, PRK, etc) and advanced cataract surgery.
Dr. Behunin was born and raised in a multi-generation coal-mining family in Clawson, Utah.
He studied pre-medicine at Southern Utah University. He obtained a Bachelor of Science degree in business administration and a Doctor of Medicine degree at the University of Utah. He completed residency training in ophthalmology at the Pennsylvania State University. He then completed a fellowship in cornea and external disease at the University of Michigan’s Kellogg Eye Center, consistently ranked among the top 7 eye centers in the US.
Dr. Behunin is a member of the American Academy of Ophthalmology, the Cornea Society, and the American Society of Cataract and Refractive Surgery. His writing has been featured in many ophthalmology textbook chapters, popular ophthalmic magazines, and in peer-reviewed journals.

Marcos Reyes, MD
Dr Reyes is board certified by the American Board of Ophthalmology. He was born in New York City and then moved to Houston Texas at an early age where he did the majority of his schooling. He attended Brigham Young University where he met and married his sweetheart, Raina. After their undergraduate studies were completed.
Dr. Reyes attended medical school at Texas Tech University Health Sciences Center. He went on to finish his Ophthalmology Residency at the University of Texas in San Antonio, Texas. During his residency he received refractive surgery training in LASIK and PRK at the nationally recognized Air Force WarFighter Refractive Surgery Center. He received many honors including the University of Texas School of Medicine “Joe R McFarlane award for Compassion, Fairness, Justice, Integrity.” He was also selected selected by his peers and attending professors to serve as the sole Chief Resident for his senior year of residency.
Following Dr. Reyes’ residency, he completed a glaucoma fellowship at the prestigious Dean McGee Eye Institute in Oklahoma City, Oklahoma. He then took a teaching position with the faculty of the University of Missouri Mason Eye Clinic as an assistant professor for 5 years, training Ophthalmology residents surgical and clinical techniques. There he was assigned as clinic director in addition to receiving a number of teaching awards.
In 2015 he and his family moved to Southern Utah to continue practicing medicine in private practice. They have four wonderful children whom he loves spending time with. Hiking, mountain biking and reading are a few of his favorite hobbies.
With over a decade of glaucoma specialty work he is very experienced in glaucoma decision making and surgery. He is an experienced LASIK, PRK, and implantable collamer lens (ICL) surgeon as well as an excellent cataract surgeon and advanced anterior segment surgery specialist.

Nathan C. Anderson, MD
Dr. Anderson is board-certified by the American Board of Ophthalmology.
Dr. Anderson was born and raised in Salt Lake county. He attended Brigham Young University, graduating with Magna Cum Laude honors with a bachelors of science in neuroscience and obtaining a minor in business management. He attended medical school at Texas Tech University Health Science Center where he was honored with membership to both the Alpha Omega Alpha Honor Society as well as the Gold Humanism Honor Society.
He completed his ophthalmology residency at Wake Forest University in Winston-Salem, North Carolina where he was elected by the faculty and residents to serve as the Chief Resident his final year. He also received the Wake Forest Outstanding Consultant Award while working there. He elected to continue his training at Wake Forest with a one-year oculoplastics fellowship under the direction of Dr. Charles Rice, MD, expanding his surgical skill set to include procedures of the eyelid and nasolacrimal duct system.
Dr. Anderson is a member of the American Academy of Ophthalmology. He and his wife, Allie, are the proud parents of four children. Joshua Terry, OD
Dr. Joshua Terry graduated from Southern California College of Optometry. He gained advanced training at the Moran Eye Center, SLC Veterans Hospital, and Phoenix area Indian Health Service, specializing in treatment of ocular disease and management of LASIK and cataract refractive surgery patients. Since graduating in 2007, Dr. Terry has always worked in ophthalmology, first working in Arizona, before moving to Utah in 2010. Dr. Terry also provides eye care services to rural Nevada through his own private practice. He is licensed by the Utah, Nevada and Arizona State Board of Optometry and is a member of the Utah and American Optometric Associations. Dr. Terry was born in Richfield, Utah and currently resides in St. George with his wife, Jamie, and their 6 boys. Dr. Terry and his wife are active advocates in early detection of Type 1 Diabetes and founded a nonprofit organization, Kisses For Kycie, after the unexpected death of their 5 year old daughter, Kycie.

Sherine Smith, PA-C
Sherine Smith is a nationally certified Physician Assistant (PA- C, MPAS) with an emphasis in Ophthalmology. She completed her undergraduate degree from Brigham Young University, majoring in Zoology/ Human Biology. After working in Ophthalmology as a technician, she went on to complete a Master’s Program in Physician Assistant studies from the University of Utah. She graduated at the top of her class and received the Humanitarian Service Award for her contributions to the community. Some of her humanitarian efforts include trips to Bolivia and Mexico to provide health care and improve living conditions. She spent 7 years practicing in Family Medicine in at a National Health Service clinic in Provo, UT. before finding her way back into Ophthalmology here at the St. George Eye Center. Her experience in family practice allows for a unique approach to how systemic illnesses can affect the eyes. She is also fluent in Spanish.

Kierstin Lealiiee is a nationally-certified Physician Assistant (PA) with licenses to practice in both Utah and Nevada. After completing her undergraduate in Physiology & Developmental Biology at Brigham Young University, Kierstin obtained a Master of Science in Physician Assistant Studies from Rocky Mountain University of Health Professions.
